Information for patients
The renal Social worker works with patients who are experiencing challenges in their daily life which are impacting on their ability to manage their dialysis/health needs.
The renal social worker travels to the various dialysis sites and can meet with patients during their dialysis sessions or within their home environments. Appointments and meetings are made on a case by case basis with the patient.
The renal social worker works with multidisciplinary teams to support patients to identify and access their desired outcomes. They have good working relationships with the surrounding local authorities, relevant third sector organisations and can access appropriate levels of advice and support for patients.

Referrals can also be made via the telephone but when leaving a message, please ensure you clearly state your full name, contact number and reason for the referral.
Patients can self-refer or ask a member of their renal team to refer to the service.
