The CUH falls prevention and management service is committed to reducing the incidence of patient falls across the trust.
We are a multiprofessional team, working collaboratively with clinical teams, patients and their carers to improve patient care and safety, to identify and manage fall risks and promote learning from patient safety incidents.
A member of the falls prevention team will be available Monday to Friday, 08:00-16:00.
We are working towards continuous quality improvement work, to ensure the care patients receive meets national benchmark standards.
If you require information on inpatient falls prevention and management or community falls prevention services please liaise with your treating team, who can also forward any queries to the falls prevention team.
Falls prevention clinic
We offer a one-stop multidisciplinary assessment to find out why an individual has fallen and formulate a individualised management plan for reduction and prevention of falls in Clinic 2, Addenbrooke’s hospital - Every Tuesday afternoon
Referral
Referrals can be made via choose and book to the Department of Medicine for the Elderly.
Inclusion criteria
Recurrent falls i.e. two or more falls in the last year
The assessment
- The assessment can take up to 1 hour and may include:
- Review by Consultant Geriatrician to discuss falls and review medication
- Therapy assessment to assess walking, mobility and muscle weakness
- Lying and standing blood pressure checks
- ECG
- Bloods tests and further investigations when needed
- Onwards referral to community service as necessary
- Full report to be sent to GPs and other agencies if required
